Explanation from dr. Jecky Chandra: The patient had a fracture and an unstable elbow

According to dr. Jecky Chandra, the patient sustained an injury after falling while participating in a sack race as part of the Independence Day celebration.

An examination showed a complex fracture of the radial head, or the head of the radius bone near the elbow, as well as a fracture of the ulna in the proximal third. The injury was also accompanied by instability of the elbow joint.

The condition made it difficult for the patient to move the elbow. Each time the elbow was moved, the patient experienced significant pain that interfered with daily activities. To treat the condition, dr. Jecky performed partial elbow joint replacement involving the radial head. In addition, pins and an anchor were placed to help maintain the stability of the elbow joint.

After the procedure, the patient’s elbow could be moved through full flexion and extension, as well as supination, or rotating the forearm so that the palm faces upward. A postoperative examination also showed that the elbow joint was stable.

When is elbow replacement surgery generally recommended?

The elbow joint can become damaged due to various conditions, ranging from arthritis to fractures and other injuries. In many cases, damage caused by arthritis or a fracture can still be repaired through surgery. However, when the damage is too severe, elbow replacement surgery may be a more appropriate treatment option.

Pain and reduced ability to move the elbow are also among the most common reasons why a person may undergo elbow replacement surgery.

Some conditions that can cause damage to the elbow joint include:

  • Various types of arthritis.
  • Fractures.
  • Bone tumors.

Types of elbow replacement surgery

There are several types of elbow replacement surgery. Total and partial elbow replacement are distinguished by how much of the elbow joint needs to be replaced. Meanwhile, linked and unlinked elbow replacements are two techniques that surgeons can use to attach the artificial joint or prosthesis.

The surgeon will determine whether a patient needs total or partial elbow replacement based on the condition and severity of the joint damage.

  • Total elbow replacement: Total elbow replacement is the most common type of elbow replacement surgery. This procedure is sometimes referred to as total elbow arthroplasty. The surgeon replaces the damaged parts of the upper arm bone (humerus) and forearm bones (ulna and radius) with a prosthesis.
  • Partial elbow replacement: As the name suggests, this procedure involves replacing only part of the elbow joint. Partial elbow replacement is much less common and is generally performed in patients who have sustained an elbow injury or trauma.

In addition to being classified according to how much of the joint is replaced, elbow replacement surgery can also be classified based on the technique used to attach the prosthesis:

  • Linked: In this technique, the surgeon inserts metal stems into the humerus and ulna to support the artificial joint. The linked technique is more commonly used.
  • Unlinked: In this technique, the artificial joint is connected to the patient’s existing ligaments and natural connective tissue. This technique is less commonly used because the patient’s ligaments are usually not strong enough to support the prosthesis. Generally, when damage or wear in the elbow is severe enough to require joint replacement surgery, the ligaments around the elbow may also be damaged.
