Explanation of the Botox injection procedure for overactive bladder by dr. Andika

According to dr. Andika, Botox or botulinum toxin is commonly known in the field of aesthetics for helping reduce wrinkles and relax facial muscles. However, medically, Botox can also be used to reduce excessive nerve and muscle activity. This mechanism is utilized in the treatment of overactive bladder or a hypersensitive bladder.

In patients with this condition, the bladder may contract excessively, causing symptoms such as frequent urination and a sudden urge to urinate that is difficult to control. Injecting Botox into the bladder muscle can help reduce this excessive activity, thereby decreasing bladder sensitivity and helping patients gain better control over urination.

The Botox injection procedure performed by dr. Andika used 200 units of Botox, which were injected into the patient’s bladder. After the procedure, the patient was observed for approximately 1–2 hours to ensure that their condition remained stable, including confirming that they could urinate normally. If no problems were found, the patient could return home on the same day, allowing the procedure to be performed under a one-day care concept.

The effects of Botox are not immediate and do not reach their maximum effect right after the procedure. The therapeutic effects generally begin to appear and develop over several days to weeks, with the maximum expected effect occurring around 2–3 weeks after the procedure. During this period, the frequency of urination is expected to decrease, while the patient’s ability to control the urge to urinate may improve.

However, the effects of Botox are temporary, so the procedure may need to be repeated if symptoms return. The interval between procedures can vary from one patient to another. In some cases, the procedure may be repeated approximately every 6–8 months, depending on the patient’s response to treatment and whether overactive bladder symptoms recur.

Benefits of Botox injections for overactive bladder

When Botox therapy for the bladder is successful, patients may experience several benefits, including:

  • Reducing or eliminating episodes of urine leakage or urinary incontinence.
  • Reducing or eliminating a sudden, strong urge to urinate that is difficult to control.
  • Reducing the frequency of urination during both the day and night.
  • Reducing or eliminating the need to use special pads to manage urine leakage.
  • Improving the patient’s quality of life.

Approximately 70–75% of patients experience a significant reduction in symptoms and an improvement in quality of life after undergoing Botox therapy for the bladder.

How is Botox injection for overactive bladder performed?

Botox injections into the bladder are generally performed as an outpatient procedure, with the following steps:

  • The patient is given local anesthesia to help reduce discomfort during the procedure.
  • Local anesthetic gel is applied or inserted into the urethra, the tube through which urine leaves the body.
  • The doctor then inserts a small, camera-equipped tube to examine the inside of the bladder. This procedure is called cystoscopy.
  • After examining the bladder, Botox is injected using a special needle into several areas of the bladder wall.

After the procedure, the patient is monitored for a period of time before being allowed to go home.
