Myasthenia Gravis

Myasthenia gravis adalah penyakit autoimun yang terjadi akibat antibodi merusak komunikasi antara otot dan saraf, sehingga otot menjadi lemah. Otot yang sering terdampak kondisi ini adalah otot mata, mulut, tenggorokan, dan kaki serta tangan.

Meski bisa dialami segala usia, pengidap myasthenia gravis paling banyak berusia 20-30 tahun pada wanita dan di atas 50 tahun untuk pria.

Kondisi ini perlu segera ditangani, karena jika otot-otot yang terdampak tidak berfungsi dengan baik, maka aktivitas harian akan sulit dilakukan. Pada kondisi yang parah, pengidap myasthenia gravis bisa mengalami kesulitan menelan bahkan bernapas. 

Myasthenia gravis tidak bisa disembuhkan. Namun, kondisi ini bisa dikontrol gejalanya. Semakin cepat terdeteksi, semakin baik.

Penyebab 

Myasthenia gravis terjadi ketika ada gangguan antara komunikasi saraf dan otot. Karena itu, penyakit ini masuk ke dalam golongan penyakit neuromuskular. 

Normalnya, saraf berkomunikasi dengan otot dengan cara mengeluarkan zat kimia yang disebut neurotransmiter. Zat ini secara alami akan masuk ke area sel otot yang disebut reseptor.

Pada orang dengan myasthenia gravis, antibodi yang seharusnya melindungi tubuh, justru menghancurkan reseptor di otot. Ini membuat semakin sedikit otot yang menerima sinyal dari saraf, sehingga otot jadi lemah dan sulit digerakkan. 

Gejala

Gejala myasthenia gravis yang umum dialami antara lain: 

  • Kelopak mata turun
  • Penglihatan kabur dan objek terlihat berbayang atau ganda
  • Kesulitan untuk tersenyum
  • Kesulitan bernapas dan menelan
  • Tangan dan kaki sangat lemah, hingga sulit digerakkan
  • Badan lemas terus menerus
  • Tidak bisa melakukan aktivitas sehari-hari 

Gejala akan semakin parah saat pengidapnya sedang merasakan kondisi-kondisi di bawah ini:

  • Kelelahan
  • Infeksi 
  • Sehabis menjalani operasi
  • Stres
  • Konsumsi obat-obatan tertentu seperti beta blocker, obat bius tertentu, dan antibiotik.
  • Hamil
  • Menstruasi

Diagnosis

Ada beberapa tes yang bisa dilakukan dokter untuk mendiagnosis myasthenia gravis, yaitu:

  1. Tes darah

Tes darah dilakukan untuk melihat jenis antibodi yang menghentikan komunikasi antara saraf dan otot. 

  1. Pemeriksaan saraf dan otot

Pemeriksaan fungsi saraf dan otot dilakukan dengan elektromiografi atau EMG. 

Pada pemeriksaan ini, dokter akan memasukkan jarum kecil ke area-area otot yang mengalami gangguan. 

Jarum tersebut akan menangkap sinyal hantaran saraf ke otot dan menggambarkannya dalam bentuk grafik di layar. Dokter spesialis neurologi akan melihat grafik yang tertera pada layar dan menilai apabila ada gangguan pada komunikasi antara saraf dan otot.

  1. Scan

CT-Scan dan MRI juga bisa dilakukan untuk mendeteksi myasthenia gravis. Pada pengidap penyakit ini, biasanya ada pembesaran thymus yang akan terlihat pada hasil scan. 

Thymus adalah kelenjar kecil pada dada yang berperan penting dalam sistem imun. 

  1. Tes genetik

Tes genetik dilakukan untuk melihat ada atau tidaknya gen yang diwariskan dari orang tua yang menyebabkan meningkatnya risiko myasthenia gravis.

Pengobatan

Penanganan Myasthenia Gravis yang bisa dilakukan adalah:

  1. Pemberian obat

Jenis obat yang diberikan antara lain anticholinesterase, steroid, atau obat-obatan untuk menekan imun (imunosupresan)

  1. Plasmaferesis

Plasma adalah cairan bening pada darah. Pada plasmaferesis, cairan plasma akan dipisahkan dari sel-sel darah lalu kemudian ditempatkan kembali ke tubuh untuk merangsang lebih banyak pembentukan plasma dibanding sebelumnya. 

  1. Immunoglobulin Intravena (IVIG) 

Immunoglobulin yang mengandung antibodi sehat dari donor diberikan secara intravena pada pasien dengan GBS. Pada dosis tinggi, immunoglobulin bisa mencegah kerusakan antibodi lebih jauh yang bisa memperparah kondisi GBS. 

Metode ini diharapkan dapat menyingkirkan antibodi tertentu yang berkontribusi pada serangan sistem imun ke saraf tepi.

  1. Operasi

Multiple Sclerosis

Multiple sclerosis (MS) is one of the muscle and nerve diseases that, in severe conditions, can lead to paralysis. This condition cannot be completely cured, but symptoms can be treated to improve the quality of life of the sufferers.

What is multiple sclerosis?

Multiple sclerosis is a chronic autoimmune disease that affects the central nervous system, including the brain and spinal cord.

This condition causes the immune system to attack the protective covering of nerve fibers (myelin), leading to inflammation and damage to the myelin. As a result, there are nerve disturbances that cause individuals to experience vision problems, mobility issues (disabilities), and loss of body balance.

Multiple sclerosis lasts a lifetime. Its severity can vary from mild to severe.

This condition can affect anyone of any age. However, multiple sclerosis most commonly occurs in women aged 20 to 40 years.

Causes of multiple sclerosis

As mentioned earlier, multiple sclerosis occurs because the body’s defense system attacks nerve fibers (autoimmune). However, the exact triggers of this autoimmune condition are not yet known.

Doctors suspect that this autoimmune reaction may be caused by:

  • Viral infections
  • Genetic abnormalities
  • Environmental factors

Symptoms of multiple sclerosis

Symptoms of multiple sclerosis can vary from person to person. However, here are some common symptoms experienced by most patients:

  • Blurry vision or double vision
  • Red-green color distortion in vision
  • Pain and vision loss due to optic nerve swelling (optic neuritis)
  • Difficulty walking and maintaining balance
  • Numbness, tingling, or prickling sensations (paresthesia)
  • Weakening of muscles in the arms and legs
  • Body coordination disturbances such as difficulty walking or standing. Some people may experience paralysis
  • Persistent fatigue
  • Difficulty speaking
  • Tremors
  • Dizziness
  • Hearing loss
  • Digestive and bladder disturbances
  • Depression
  • Changes in sexual function
  • Decreased concentration, attention, memory, and the ability to make sound judgments.

Treatment of multiple sclerosis

Currently, there is no cure for multiple sclerosis. However, doctors have several ways to alleviate symptoms and improve the quality of life of patients, such as:

1. Medications

Doctors can prescribe medications that suppress the immune response. The goal of these medications is to reduce inflammation caused by the autoimmune response.

This method also helps slow down the damage to myelin. Medications are usually administered through infusions.

In addition to medications to combat inflammation, doctors can also prescribe anticonvulsants and medications that can maintain cognitive function.

2. Rehabilitation therapy

Physiotherapy, occupational therapy, and speech therapy may be recommended for patients experiencing movement and speech disorders.

3. Counseling

Counseling aims to provide encouragement to patients to prevent depression due to multiple sclerosis.

Diagnosis

The diagnosis of multiple sclerosis requires several tests such as:

  • MRI. This test is performed to look for lesions on myelin around the brain and spinal cord. The presence of lesions may indicate multiple sclerosis.
  • Electromyogram (EMG) test. This test records the brain’s electrical responses to visual, auditory, and sensory stimuli. This test indicates whether you are experiencing message slowdowns in various parts of the brain.
  • Cerebrospinal fluid analysis. Also known as a spinal tap. This method aims to examine cellular and chemical abnormalities due to multiple sclerosis.
  • Blood tests. Tests are performed to confirm that symptoms originate from multiple sclerosis and not from other diseases.
  • Eye examination. To see if vision disturbances are caused by other conditions.
    Prevention of multiple sclerosis

Unfortunately, there is no definitive way to prevent multiple sclerosis. This is because the exact cause is not yet known. However, consuming healthy foods and leading a healthy lifestyle are believed to reduce the risk of this disease.

    Excellent urologist and a very kind person, a combination any patient would want in a doctor. Detailed and thorough when explaining the treatment I needed to have during initial consultation. Had RIRS to remove 0.75 cm kidney stone and removal of stent performed by him. Skilled and knowledgeable in the latest technology needed for the procedure. Procedure took 1 hour, kept overnight and was discharged the next afternoon after visitation. Before being discharged he went over and showed evidence of the successful procedure and advised me on how to maintain a stone free kidney. Very professional and meticulous in his approach to taking care of the patient. Hospital’s services was excellent too during my stay and the food is restaurant standard.

    Before being discharged he explained how the procedure went, showed evidence of the successful procedure and educated me how to maintain a stone free kidney.
    Very responsible and professional in his approach towards taking care of his patients.
    Hospital is excellent as well with its services during my short stay and the food is restaurant quality.
