The BERA test (Brainstem Evoked Response Audiometry) is a diagnostic procedure used to assess how the brain processes sound. This test records brain wave activity in response to auditory stimuli such as clicking sounds or specific tones.

It is particularly useful for diagnosing hearing problems or nervous system disorders in newborns and children.

Purpose of the BERA Test

The BERA test is typically performed to:

  • Help diagnose neurological disorders and hearing problems, especially in infants and children
  • Evaluate the functional integrity of the auditory nervous system
  • Assess hearing in individuals who are unable to participate in conventional hearing tests

In some cases, this test is also used during surgeries to minimize the risk of damage to the auditory nerves and brain.

BERA Test Procedure

The BERA test uses electrodes attached to the scalp to measure the brain’s response to repetitive auditory stimulation. These stimuli usually consist of short, sharp clicking sounds.

First introduced in 1971, the BERA test is considered the gold standard for hearing evaluation in infants and young children. It is also widely used in neurosurgery, such as in cases of inner ear tumor removal.

Before the Procedure

For adults, doctors typically advise washing their hair the night before the test to ensure proper electrode attachment.

For children, a sedative may be administered to help them stay relaxed and still during the test.

During the Procedure

The patient will lie down on a reclining chair or bed and must remain still throughout the process. Electrodes are then attached to the scalp and behind each ear. These electrodes are connected to a machine that records brain activity.

Next, clicking sounds or short tones are played through earphones. The electrodes detect and record the brain’s response to these sounds.

BERA Test Results

The BERA test results are presented as a visual graph showing brain wave activity. If the graph shows a flat line when certain tones or clicks are played, it may indicate hearing loss.

Abnormal BERA results may also suggest damage to the brain or nervous system, potentially due to:

  • Multiple sclerosis (an autoimmune disease that damages the protective sheath around nerve cells)
  • Central pontine myelinolysis (a disorder that affects the myelin sheath of nerve cells)
  • Acoustic neuroma (a tumor on the nerve connecting the ear to the brain)
  • Stroke
  • Brain injury
  • Brain tumor
  • Speech disorders

If the test results are abnormal, further diagnostic tests may be required to determine the exact cause. Once identified, the doctor will discuss the appropriate treatment options with the patient.
