Understanding Adenomyosis: Causes, Symptoms, and HIFU Treatment at Mandaya Royal Hospital Puri

Mengenal Adenomiosis: Penyebab, Gejala, hingga Penanganannya dengan HIFU di RS Mandaya Puri

Adenomyosis is a condition in which tissue similar to the inner lining of the uterus (endometrium) grows into the muscular wall of the uterus (myometrium). This can cause the uterus to thicken and enlarge, sometimes up to two to three times its normal size. Adenomyosis often leads to symptoms such as painful menstruation, heavy or prolonged menstrual bleeding with blood clots, and pain in the abdominal or pelvic area.

Many women are unaware that they have adenomyosis because the condition does not always cause noticeable symptoms. To date, the exact prevalence of adenomyosis is not clearly known. However, it is more commonly found in women over the age of 40 and those who have undergone uterine procedures. Additionally, about 2–5% of adolescents who experience severe menstrual pain are known to have adenomyosis.

At Mandaya Royal Hospital Puri, adenomyosis can be treated using High-Intensity Focused Ultrasound (HIFU), a non-surgical and incision-free procedure.

What are the symptoms of adenomyosis?

Approximately 1–3 individuals with adenomyosis may not experience any symptoms. However, others may develop:

  • Severe menstrual pain (intense cramps)
  • Heavy menstrual bleeding
  • Irregular menstrual cycles
  • Pelvic pain, with or without severe cramps
  • Pain during sexual intercourse
  • Difficulty getting pregnant (infertility)
  • Enlarged uterus
  • A feeling of bloating or fullness in the abdomen

What causes adenomyosis?

The exact cause of adenomyosis remains unknown. However, several factors are believed to contribute, including hormonal influences, genetic predisposition, inflammation, and a history of uterine injury.

Risk factors for adenomyosis include:

  • Age between 40–50 years
  • Having given birth at least once
  • History of uterine procedures, such as fibroid removal surgery or curettage
  • Having endometriosis

However, adenomyosis is increasingly being diagnosed in women in their 30s, particularly those experiencing abnormal bleeding or severe menstrual pain.

What are the complications of adenomyosis?

Symptoms of adenomyosis tend to worsen over time. Heavy menstrual bleeding can increase the risk of anemia, a condition in which the body lacks sufficient red blood cells containing iron. This may cause fatigue and increased sensitivity to cold.

If left untreated, adenomyosis may also increase the risk of infertility or miscarriage, as it can make it difficult for an embryo to implant in the uterine lining.

Additionally, the condition can cause chronic pelvic and abdominal pain that interferes with daily activities.

Adenomyosis itself does not cause cancer and does not develop into cancer.

Adenomyosis treatment with HIFU at Mandaya Royal Hospital Puri

Mandaya Royal Hospital Puri offers HIFU technology for the treatment of adenomyosis. HIFU is a medical procedure that uses high-intensity focused sound waves to treat various conditions, including adenomyosis. Unlike ultrasound imaging (USG), which is used for diagnostic purposes, HIFU utilizes focused ultrasound waves to destroy targeted tissue.

This procedure is minimally invasive as it does not require any incisions, making it a more comfortable treatment alternative for patients.

The mechanism of HIFU can be compared to a magnifying glass focusing sunlight onto a single point. The ultrasound waves are precisely directed to the target area, generating heat that destroys abnormal tissue without damaging surrounding healthy tissue. These waves can also penetrate the skin without causing injury. During the procedure, doctors typically use MRI or ultrasound guidance to ensure accuracy, safety, and precision.

Advantages of HIFU include:

  • No incisions and no radiation
  • High precision targeting without damaging surrounding healthy tissue
  • Usually performed as an outpatient procedure with a relatively fast recovery time
  • Lower risk of side effects compared to more invasive methods such as open surgery, chemotherapy, or radiation therapy
