Prostate Cancer Treatment Options by Prof. Chaidir at Mandaya Royal Hospital Puri

Pilihan Pengobatan Kanker Prostat oleh Prof. Chaidir di RS Mandaya Royal Puri

Prostate cancer is one of the most common cancers affecting men, especially those over the age of 50. It develops when cells in the prostate gland grow uncontrollably and may cause symptoms such as frequent urination (particularly at night), weak urine flow, painful urination, or blood in the urine or semen. In some cases, prostate cancer shows no symptoms in its early stages, leading many patients to seek care only after the disease has progressed.

To help the public better understand safe and effective treatment options, renowned Indonesian urologist and practicing doctor at Mandaya Royal Hospital Puri, Prof. dr. Chaidir Arif Mochtar, Sp.U(K), provides a complete explanation of the different therapies available based on the patient’s condition and cancer stage.

Prostate Cancer Treatment Options Explained by Prof. Chaidir

1. Radical Prostatectomy

Radical prostatectomy is a surgical procedure to remove the entire prostate gland and surrounding tissues. According to Prof. Chaidir, this procedure has evolved from open surgery to laparoscopy about 20 years ago, and is now entering the era of robotic-assisted surgery, which offers more precision and faster recovery. This method is typically recommended for localized prostate cancer.

2. Radiotherapy

Radiotherapy is a key treatment for locally advanced prostate cancer—when the cancer has slightly spread beyond the prostate and surgery may leave residual cancer cells. Radiation helps destroy cancer cells that cannot be removed surgically and is an effective alternative for certain stages.

3. Chemotherapy

For patients whose prostate cancer has metastasized (spread to the bones, lungs, or lymph nodes), chemotherapy is one of the systemic therapies commonly used.

4. Immunotherapy

Immunotherapy has shown promising results in cancers such as breast, lung, and kidney. However, based on the latest studies (2025), immunotherapy for prostate cancer has not yet demonstrated significant benefits compared to standard treatments. While the potential remains high, further research is needed for its use in prostate cancer.

5. PSMA Nuclear Therapy (PSMA-Targeted Therapy / Theranostics)

This is one of the latest and most promising innovations. The therapy uses PSMA (Prostate-Specific Membrane Antigen) as a “targeting guide” to deliver radioactive isotopes such as Lutetium-177 directly to prostate cancer cells throughout the body. The treatment begins with a PSMA PET Scan to locate cancer cells, followed by an injection of Lutetium-177 to destroy them. Although currently limited to specific stages, ongoing research aims to expand its use to a wider range of cases.
