Mandaya Royal Hospital Puri, the First and Only Hospital in Indonesia Offering HIPEC Chemotherapy

Kemoterapi HIPEC Pertama dan Satu-satunya di Indonesia Ada di RS Mandaya Royal Puri

Mandaya Royal Hospital Puri is the first and only hospital in Indonesia to provide HIPEC (Hyperthermic Intraperitoneal Chemotherapy) for treating advanced-stage colon cancer and ovarian cancer that has metastasized to the abdominal cavity. This innovative cancer therapy has also been implemented in several leading hospitals in Singapore and Malaysia.

Compared to conventional chemotherapy, HIPEC offers several distinct advantages. Moreover, the procedure is performed in a single session, unlike systemic chemotherapy that requires multiple treatment cycles.

Understanding HIPEC Chemotherapy and Its Procedure at Mandaya Royal Hospital Puri

HIPEC is an advanced cancer treatment specifically designed to target tumors growing inside the abdominal cavity (peritoneum)—the area that protects vital organs such as the liver, stomach, and intestines.

During the procedure, surgeons first remove any visible tumors. Then, a heated chemotherapy solution is circulated within the abdominal cavity. The combination of heat and direct exposure to chemotherapy drugs effectively destroys microscopic cancer cells that may remain after surgery.

The HIPEC procedure consists of two main stages performed within one surgical session: cytoreductive surgery and HIPEC chemotherapy itself.

1. Cytoreductive Surgery Stage

In this stage, the surgeon removes as much visible tumor tissue as possible from the abdominal cavity.
Under general anesthesia, the surgeon opens the abdominal area to examine the extent of cancer spread. Any infected tissues or organs are carefully removed to minimize tumor burden. This step ensures that the heated chemotherapy administered afterward can work more effectively to eliminate remaining cancer cells.

2. HIPEC Chemotherapy Stage

After all visible tumors are removed, the procedure continues with high-temperature intraperitoneal chemotherapy. A special catheter is inserted into the abdominal cavity and connected to a perfusion machine that heats the chemotherapy solution to around 40–50°C. The heated solution is then circulated for 1–2 hours, allowing even exposure throughout the peritoneal cavity to kill microscopic cancer cells that may not be visible during surgery.

Once circulation is complete, the chemotherapy fluid is drained, and the abdominal cavity is rinsed with a sterile saline solution. The surgery then concludes with the careful closure of the incision by the surgical team.

Advantages of HIPEC Chemotherapy

HIPEC chemotherapy provides numerous benefits for patients with advanced colon or ovarian cancer that has spread to the abdominal cavity:

  • Reaches areas that systemic chemotherapy cannot, as the heated drugs are delivered directly into the peritoneal cavity.
  • Targets and destroys microscopic cancer cells more precisely and effectively.
  • Uses high-temperature chemotherapy (40–50°C) to enhance drug penetration without increasing side effects on other organs.
  • Performed in a single surgical session, making it more efficient than systemic chemotherapy, which requires multiple treatment cycles.
  • Causes fewer side effects since the drugs act locally in the abdominal cavity and are minimally absorbed into the bloodstream.
  • Improves survival rates and reduces recurrence risk for patients with advanced-stage cancers that have spread within the peritoneal cavity.
