Uterine Fibroid Treatment for Fibroids Measuring 3.7 x 4.3 cm Using HIFU Without Surgery by dr. Widya, Sp.OG, at Mandaya Puri Hospital

Pengobatan Miom Rahim Ukuran 3,7 x 4,3 Cm dengan HIFU Tanpa Operasi oleh dr. Widya Sp.OG di RS Mandaya Puri

Uterine fibroids are benign tumors that grow inside the uterus or within its wall. They are relatively common and do not always cause symptoms. However, in some women, fibroids can cause heavy menstrual bleeding, back pain, frequent urination, and pain during sexual intercourse. Small fibroids that do not cause symptoms may not require treatment, while larger fibroids or those that cause symptoms may need medical intervention.

Today, treating uterine fibroids does not always require surgery. For patients who meet specific medical criteria, High-Intensity Focused Ultrasound (HIFU) can be an option for treatment without incisions or surgery. The suitability of this method depends on the size, location, and number of fibroids, as well as the patient’s overall health and individual needs.

One example is the treatment of a 34-year-old patient at Mandaya Royal Puri Hospital by dr. Widya Solichin, Sp.OG. The unmarried patient was found to have multiple uterine fibroids during a medical check-up (MCU). The largest fibroid measured 3.7 x 4.3 cm and was located at the back of the uterus, while the other fibroids measured 1.3 cm and 1.5 cm. The doctor then performed HIFU to treat the fibroids without surgery.

Treating Multiple Fibroids at Once With HIFU Without Incisions or Surgery

According to dr. Widya, the 34-year-old patient initially did not experience any significant symptoms. Several uterine fibroids were discovered incidentally during an MCU ultrasound examination. To obtain a more detailed assessment of the fibroids, the patient subsequently underwent an MRI scan.

The examination revealed multiple uterine fibroids, including the largest one, measuring 3.7 x 4.3 cm, located at the back of the uterus. Other fibroids measured 1.3 cm and 1.5 cm. The doctor also identified several fibroids in the front of the uterus.

After considering the patient’s condition, dr. Widya performed HIFU as a treatment method that does not require incisions. Following the procedure, the evaluation results presented by the doctor showed changes in the targeted fibroid tissue.

After the procedure, the patient experienced only mild abdominal discomfort and no significant pain. According to dr. Widya, the patient’s condition remained stable, and she was able to resume her activities after recovery.

Although the procedure had been completed, follow-up monitoring remained necessary to evaluate changes in the fibroids. The patient was scheduled to undergo repeat MRI examinations at three months, six months, and one year after the procedure to assess changes in fibroid size over time.

Understanding HIFU: Technology for Treating Fibroids Without Incisions or Surgery

HIFU is a treatment method for uterine fibroids that uses focused, high-intensity ultrasound waves to destroy fibroid tissue without requiring incisions.

Unlike conventional ultrasound, which is used to produce images of internal organs, HIFU concentrates ultrasound energy on the targeted fibroid tissue. This energy raises the temperature in the targeted area, causing tissue damage that can lead to gradual shrinkage of the fibroid.

These ultrasound waves can pass through surrounding tissues, such as the skin, without directly damaging them when the procedure is performed appropriately. During treatment, doctors can use imaging guidance, such as MRI or ultrasound, to identify the fibroid’s location and help monitor the treatment process.

Although HIFU does not require incisions or open surgery, it is not suitable for every patient with uterine fibroids. Doctors need to evaluate the size, number, and location of the fibroids, as well as the patient’s overall health, to determine whether this method is an appropriate treatment option.

Advantages of HIFU for Treating Uterine Fibroids

HIFU offers several advantages as a treatment option for uterine fibroids in patients who meet the relevant medical criteria, including:

  • No surgical incisions. HIFU uses focused ultrasound waves to treat fibroid tissue without requiring incisions.
  • Targeted treatment of fibroid tissue. Ultrasound energy is focused on the fibroid tissue to help destroy it, with the aim of minimizing the impact on surrounding healthy tissue.
  • Relatively short recovery time. HIFU can generally be performed without hospitalization, potentially allowing patients to resume their activities within a relatively short period. Recovery time still depends on the patient’s condition.
  • No radiation involved. HIFU uses high-intensity sound waves to treat fibroid tissue rather than radiation.

Symptoms of Uterine Fibroids That Often Go Undetected

Uterine fibroids often do not cause symptoms, so some women only discover them during health examinations, such as ultrasound scans. Therefore, regular health check-ups are important for helping detect fibroids, particularly when symptoms suggestive of this condition are present. Symptoms can vary depending on the size, number, and location of the fibroids in or around the uterus.

Common symptoms of uterine fibroids may include:

  • Heavier or more painful menstrual bleeding.
  • Longer menstrual periods or more frequent periods.
  • Pelvic pain or pressure.
  • Frequent urination or difficulty emptying the bladder.
  • An enlarged abdomen.
  • Constipation.
  • Lower abdominal or back pain, including pain during sexual intercourse.
  • Fatigue and weakness, which may be associated with anemia caused by heavy menstrual bleeding.

In rare cases, fibroids can cause sudden, severe pain when their blood supply is disrupted.

When Should You See a Doctor?

Consult a doctor promptly if you experience persistent pelvic pain, very heavy or painful menstrual periods that interfere with daily activities, bleeding between periods, or difficulty emptying your bladder. If you experience extremely heavy vaginal bleeding or sudden, sharp pelvic pain, seek medical attention immediately.
