A pinched nerve in the lower spine can cause severe pain radiating to the buttocks, thighs, calves, and even the soles of the feet. This condition can significantly interfere with daily activities, especially when standing or walking for long periods. In some cases, patients are forced to limit their activities because the pain occurs almost every day.

This was experienced by a 63-year-old patient named Lou Ci Fung, who suffered from pinched nerves at the L4-L5 and L5-S1 spinal levels. Before seeking treatment in Indonesia, the patient had traveled to Malaysia for medical care. However, despite undergoing therapy and medication, the symptoms did not improve.

Eventually, the patient sought a second opinion at RS Mandaya Royal Puri and underwent minimally invasive arthrospine surgery with dr. Christian Ariono, Sp.BS-FTB, FINSS, FINPS, FICS.

“Right now, her condition is good. She no longer experiences cramps or pain and feels much better. Hopefully, the cramps won’t come back in the future,” said the patient’s child while describing the patient’s postoperative condition.

Pain Radiating from the Lower Back to the Legs, Even After Treatment in Malaysia

The patient said the pain had been bothering her for around six months. The pain started from the soles of the feet and radiated up to the hips and buttocks.

“My legs hurt from the soles of my feet all the way to my buttocks and hips. Standing hurts every day, walking hurts too. I can still walk, but not for long distances,” said the patient.

The symptoms significantly limited daily activities. The patient spent most of the day sitting because the pain would worsen when standing or walking for too long.

Previously, the patient underwent an MRI examination, which showed a pinched nerve condition. Conservative treatments, including therapy and medication, were initially attempted. Since the symptoms did not improve, the patient continued treatment in Kuala Lumpur, Malaysia.

However, doctors there also recommended continuing conservative therapy first. Unfortunately, the patient still did not experience significant improvement.

On the other hand, several doctors had previously recommended spinal fixation with screws. Because the patient did not want to undergo screw fixation surgery, she eventually sought a second opinion at RS Mandaya Royal Puri.

Minimally Invasive Arthrospine Surgery with a Single Small Incision

According to dr. Christian Ariono, Sp.BS-FTB, FINSS, FINPS, FICS, the patient experienced pain on the left side of the lower back radiating to the left thigh, calf, ankle, and toes.

“The patient had been experiencing symptoms for nearly six months. She had already visited several doctors, and almost all of them recommended fixation or screw placement,” explained dr. Christian.

Because the patient refused screw fixation, dr. Christian performed a minimally invasive arthrospine procedure consisting of endoscopic lumbar decompression and lumbar discectomy.

This spinal surgery technique uses an endoscopic approach and requires only a small incision measuring around 1.5 to 2 cm.

“In this patient, we were even able to reach two levels, L4-L5 and L5-S1, using only a single incision,” said dr. Christian.

Advantages of Arthrospine Surgery for Pinched Nerves

  • Small incision and minimally invasive approach
  • Less postoperative pain
  • Faster recovery
  • Lower risk of bleeding and infection
  • Minimal muscle and tissue damage
  • Clearer and more precise nerve visualization
  • Smaller surgical scars
  • Shorter hospital stay

Significant Improvement After Surgery

After undergoing minimally invasive arthrospine surgery at RS Mandaya Royal Puri, the patient reported that her condition had improved significantly compared to before surgery.

“Now she’s doing well. She no longer experiences cramps or pain and feels much better,” said the patient’s family.

The significant improvement allowed the patient to return to daily activities more comfortably without the constant pain and cramps that had previously occurred almost every day.

Patients experiencing lower back pain radiating to the legs, numbness, tingling, or weakness in the legs that does not improve are advised to consult a neurosurgeon for further evaluation and appropriate treatment.
