Kidney Transplant Preparation Involves Multiple Medical Teams

A kidney transplant is not a procedure that can be performed instantly. Before the surgery takes place, patients must go through several preparation stages involving multiple medical disciplines.

Dr. dr. Maruhum Bonar H. Marbun, Sp.PD-KGH, FINASIM, an internal medicine specialist and kidney-hypertension consultant (nephrologist) at RS Mandaya Royal Puri, explained that the kidney transplant process begins with advocacy and a comprehensive evaluation by multiple medical teams.

From the nephrology perspective, the patient’s kidney condition is thoroughly assessed, including dialysis preparation, fluid balance, and the possibility of fluid overload. This evaluation is important to determine the patient’s readiness for transplantation and the administration of immunosuppressive medications after surgery.

Meanwhile, from the urology perspective, doctors assess the patient’s tolerance for surgery, including previous surgical history and the condition of blood vessels that will be used during the transplant procedure.

The radiology team also plays an important role in determining which blood vessels are most suitable for the transplant. In addition, the condition of the heart, lungs, and other organs is thoroughly evaluated to ensure the patient can safely undergo the procedure.

With careful preparation across various medical specialties, the kidney transplant can be performed safely and in a well-controlled manner.

Surgical Process: Donor Kidney Retrieved Using Laparoscopic Technique

In this transplant procedure, the left donor kidney along with its ureter was selected. The kidney was retrieved using a laparoscopic technique, a minimally invasive surgical method that involves small incisions.

The procedure was performed through the back of the body via the retroperitoneal approach. Using this technique, the donor kidney was successfully removed in a relatively short time without significant complications.

After the kidney was removed, it was immediately placed in a special ice solution to maintain optimal organ condition. A preservation fluid was then circulated through the kidney’s blood vessels to flush out any remaining blood within the organ.

Approximately eight minutes after removal, the donor kidney was transferred to the operating room where the recipient was located for the transplantation process.

The recipient team then prepared the kidney’s blood vessels, including the artery and vein, before connecting them to the patient’s blood vessels. The donor kidney’s artery and vein were then carefully attached to the recipient’s artery and vein with precision.

Once blood flow was restored, the recipient’s blood began circulating through the donor kidney, allowing the organ to resume its function of filtering the blood.

The result of this filtration process is urine, which flows through the donor kidney’s ureter and is then connected to the patient’s bladder.

Kidney Function Observed Immediately and Patient Monitored in ICU

One of the early signs of a successful kidney transplant is when the donor kidney begins producing urine after blood flow has been restored.

The medical team then performed further examinations using ultrasound (USG) to ensure that blood flow was properly distributed throughout the kidney tissue.

After the surgery was completed, the patient was transferred to the Intensive Care Unit (ICU) for close monitoring over several days. During this period, doctors carefully observed the function of the new kidney as well as the patient’s urine production.

Monitoring is typically carried out for 1–3 days, or longer if necessary, to ensure the patient remains stable after surgery.
