An irregular heartbeat, medically known as arrhythmia, is a condition in which the heart does not beat in a normal rhythm. This disorder can occur due to problems in certain parts of the heart, causing the heartbeat to become too fast, too slow, or completely irregular.

2D & 3D Cardiac Ablation
Mandaya Royal Hospital Puri provides cardiac ablation services using conventional 2D technology as well as advanced 3D cardiac ablation to treat heart rhythm disorders. In both 2D and 3D ablation methods, the arrhythmia trigger areas are treated gradually because they still use a single-point catheter, meaning each abnormal point is corrected one by one over a duration of approximately 3–4 hours.
Both 2D and 3D cardiac ablation utilize heat energy known as radiofrequency ablation (RFA) or cold energy, also referred to as cryoablation. The choice of energy type is tailored to each patient’s condition and medical needs. The catheter is inserted through a blood vessel in the groin area and guided toward the heart. Once it reaches the target location, heat or cold energy is delivered to create small lesions that block abnormal electrical pathways in the heart.
“With 3D cardiac ablation, it greatly helps guide us as doctors and operators to locate (map) the source of abnormal heart rhythms more precisely and accurately,” said dr. Sebastian.
“This ablation catheter provides one of the answers in treating arrhythmia patients because it offers very promising outcomes. Around 93–95% of patients are completely cured, with no recurrence or relapse,” he added.

Cardiac Ablation Using the PFA Method
Unlike 2D and 3D cardiac ablation, which use heat or cold energy, Mandaya Royal Hospital Puri also offers cardiac ablation using the PFA method, which is specifically designed to treat atrial fibrillation (AFib) arrhythmia with various advantages. In general, the initial stages of the PFA procedure are similar to conventional ablation, where the catheter is inserted through the groin and guided to the heart.
The main difference lies in the type of catheter and energy used. In the PFA method, doctors use a multipoint or multipolar catheter capable of delivering energy to multiple points simultaneously. The energy used is not heat or cold, but electrical energy, making the procedure significantly more efficient and shorter compared to 2D and 3D cardiac ablation.
“In PFA, the catheter used is a multipolar catheter, allowing energy to be delivered simultaneously,” explained dr. Sebastian.
“Because it delivers a greater amount of ablation energy, the procedure for atrial fibrillation ablation can be much shorter. With 3D RFA, the procedure can take 3–4 hours, whereas with PFA it can be completed in less than 2 hours,” he emphasized.
In addition to a shorter procedure duration, PFA cardiac ablation is also known to have a higher long-term success rate in treating atrial fibrillation arrhythmia compared to other cardiac ablation methods.
