In general, the stages of the ACDF procedure include:

  • On the day of surgery, the patient is given general anesthesia by an anesthesiologist so that they are fully asleep and feel no pain during the procedure. The surgeon then makes a small incision approximately 2–5 cm long on the front of the neck, usually horizontally on one side.
  • The airway (trachea) and esophagus are gently moved aside to provide access to the cervical spine. The surgeon then removes the problematic intervertebral disc and any bone spurs that are compressing the nerves. In certain conditions, part of the front portion of the cervical vertebra may also be removed as medically indicated.
  • The next stage is the fusion process, in which two or more cervical vertebrae are fused together to improve stability. In the space where the disc has been removed, a bone graft or a hollow metal cage filled with bone fragments—generally sourced from a bone bank—is placed.
  • To maintain stability during the bone fusion process, metal plates and screws are installed on the front of the cervical spine. Once all structures are properly positioned, the incision is closed, and the ACDF procedure is completed.
