Symptoms of a pinched nerve often begin with mild lower back pain but can gradually progress into severe discomfort that interferes with daily activities. This was experienced by a 53-year-old patient from Serang who suffered from lower back pain for approximately one month. The pain intensified during heavy activities, walking, and even while sitting, significantly limiting his mobility.
Previously, the patient experienced intermittent mild pain. However, at one point, the symptoms suddenly worsened with much greater intensity. Frequent physical exertion and heavy lifting were suspected to be the main triggers that aggravated his condition.

MRI Results Revealed L4–L5 Pinched Nerve
Following further evaluation using MRI, it was found that there was a protrusion of the spinal disc at the L4–L5 level. This condition was also accompanied by a tear in the disc wall, which was concluded to be the primary cause of the patient’s lower back pain. Medically, this condition is known as a herniated nucleus pulposus (HNP) at L4–L5, commonly referred to as a pinched nerve.
Since the disc protrusion was classified as mild to moderate and had not yet reached a severe stage, the patient was considered eligible for non-surgical treatment options.
Plasma Nucleopasty as a Non-Surgical Solution
Based on medical evaluation, the patient underwent a plasma nucleopasty procedure at Mandaya Royal Puri Hospital. This procedure is a minimally invasive treatment designed to reduce nerve compression by shrinking the protruding spinal disc.
The procedure was performed using a specialized needle guided by C-ARM radiology imaging to ensure precise needle placement at the L4–L5 level. No surgical incision was required, and general anesthesia was not used, resulting in lower risk and faster recovery.
Patient’s Condition After Plasma Nucleopasty
After undergoing plasma nucleopasty, the patient reported significant improvement. Lower back and hip pain decreased drastically, his body felt lighter, and he was able to walk comfortably without discomfort. By the next day, his condition continued to improve, and his mobility was fully restored, allowing him to return home without prolonged hospitalization.
dr. Christian Ariono, SpBS-FTB, FINSS, FINPS, FICS, the neurosurgeon who performed the procedure at Mandaya Royal Puri Hospital, explained the medical considerations behind the treatment:
“We performed this procedure because the disc protrusion was still in the moderate stage and had not yet become severe. Therefore, we could pursue non-operative pain management, which we call plasma nucleoplasty,” said dr. Christian.
“This is a non-surgical technique. We do not use a scalpel, and the patient is not under general anesthesia. During the procedure, I need to communicate with the patient to obtain real-time feedback and ensure no important nerves are affected. We only use a needle guided by C-ARM imaging,” he added.
Medical Services and Facilities at Mandaya Royal Puri Hospital
In addition to the positive treatment outcome, the patient also expressed satisfaction with the services at Mandaya Royal Puri Hospital. From clear and friendly communication by the doctor, responsive medical staff, to clean and comfortable hospital facilities, the entire treatment process—including meal arrangements and insurance services—was well-organized and seamless.
This patient’s experience demonstrates that mild to moderate L4–L5 pinched nerve conditions can be effectively treated without major surgery through plasma nucleopasty technology. With proper treatment, pain can be significantly reduced, mobility restored, and patients can resume daily activities with improved quality of life.
If you are experiencing persistent lower back pain or suspect a pinched nerve, consulting a neurosurgeon at Mandaya Royal Puri Hospital can be an important first step toward receiving appropriate treatment tailored to your condition.
