What are the symptoms of hypospadias in babies?

The main symptom of hypospadias is that the urethral opening is located on the underside of the penis rather than at the tip. In most cases, the opening is near the head of the penis, but in more severe cases, it may be located in the middle or base of the penis—or in rare instances, near or below the scrotum.

Common signs and symptoms of hypospadias in babies include:

  • Urethral opening located underneath or along the shaft of the penis, rather than at the tip.
  • Downward curvature of the penis (chordee), which may become more noticeable during erection.
  • Foreskin that doesn’t cover the entire penis, making the top appear as if it has a “hood.”
  • Abnormal urine stream direction, such as urine flowing sideways or downward.

Most babies with hypospadias can be identified shortly after birth, especially during hospital examinations. However, in mild cases, the misplaced urethral opening may be harder to detect. Parents are advised to consult a doctor if their baby’s penis looks unusual or if there are difficulties with urination.

If left untreated, hypospadias can lead to several complications, such as:

  • Abnormal penis shape, as the urethral opening is not at the tip.
  • Difficulty urinating, such as urine stream not being straight or directed downward.
  • Curvature of the penis during erection (chordee), which can cause discomfort.
  • Ejaculation problems, where semen cannot be released normally, potentially affecting fertility.
