Purpose of the Bentall Procedure
A doctor may recommend the Bentall procedure for patients with certain aortic conditions, such as:
- Aortic regurgitation – when the aortic valve does not close properly
- Marfan syndrome – a genetic disorder that weakens the aortic wall
- Aortic aneurysm – abnormal widening or enlargement of the aorta
- Aortic dissection – a serious condition in which the inner layer of the aorta tears
Preparation Before the Bentall Procedure
About two weeks before surgery, patients need to make several important preparations:
- Consult with your doctor about any medications you are taking, as some may need to be stopped before surgery. If taking SGLT2 diabetes medication, discontinue at least three days prior to surgery.
- Prepare your home for recovery, including arranging transportation from the hospital, stocking groceries, preparing meals, cleaning, and arranging for help with driving.
- Plan your return trip home by car or airplane according to post–heart surgery travel guidelines.
- Undergo medical tests such as ECG, chest X-ray, blood tests, nasal and groin swabs, and carotid Doppler ultrasound.
- Bathe with antibacterial soap as recommended by your doctor.
- The day before surgery, ensure you have a bowel movement, pack personal items like toiletries and glasses, and stop eating and drinking from midnight onwards.
- On the day of surgery, fast completely, shave hair from the chest, arms, legs, and groin to prevent infection, bathe again with the provided antibacterial soap, wear the hospital gown, and remove makeup, nail polish, and jewelry (dentures can be kept).
During the Procedure
The Bentall procedure is performed in a hospital operating room. Before surgery begins, patients are transported on a stretcher or wheelchair to the operating room and positioned on a narrow surgical table.
General anesthesia is administered, ensuring the patient is fully asleep and relaxed before the procedure. Once the patient is under anesthesia, the surgeon will:
- Remove the damaged portion of the aorta and the aortic valve
- Temporarily detach the coronary arteries
- Insert an artificial aortic graft with a built-in valve
- Create two openings in the graft and reconnect the coronary arteries
The entire surgery typically takes around five hours.
After the Procedure
Following the Bentall procedure, patients are monitored in the Intensive Care Unit (ICU) for 1–2 days. During this time, vital signs such as heart rate, blood pressure, body temperature, and breathing are closely observed.
The surgical wound may cause discomfort, but pain medication will be provided. Drainage tubes in the chest will help remove excess fluids.
By the first day after surgery, most patients can sit in a chair and take a few steps, but activity should be gradual to ensure proper recovery. Once stable, patients are transferred to the cardiac ward for several more days of monitoring and care.
