Hip replacement surgery is a procedure to remove damaged parts of the hip joint and replace them. The replacement part, known as a prosthesis, is typically made of metal, ceramic, and hard plastic. Its function is to reduce pain and improve hip function.

Hip replacement surgery is also known as total hip arthroplasty. It may be recommended if hip pain interferes with daily activities and nonsurgical treatments are ineffective or no longer provide relief. Arthritis is the most common reason for hip replacement surgery.

Purpose of Hip Replacement Surgery

The following are the goals and indications for hip replacement surgery:

  • Osteoarthritis: This condition damages the smooth cartilage covering the ends of the bones, which helps joints move easily.
  • Rheumatoid arthritis: Caused by an overactive immune system, this condition leads to inflammation that can erode cartilage and sometimes the underlying bone, causing joint damage or deformity.
  • Osteonecrosis: When there is not enough blood supply to the ball part of the hip joint, the bone may collapse and become misshapen. Osteonecrosis can result from dislocation or fracture that disrupts blood flow.

Hip replacement may also be considered if hip pain:

  • Persists despite taking pain relievers
  • Worsens while walking, even when using a cane or walker
  • Disturbs sleep
  • Affects the ability to climb stairs
  • Makes it difficult to get up from a seated position

Hip Replacement Procedure

Before the Procedure

Before surgery, the patient will have a consultation with an orthopedic surgeon to:

  • Review medical history and current medications
  • Examine the hip, checking joint range of motion and surrounding muscle strength
  • Request blood tests and X-rays; MRIs are rarely needed

The doctor may also advise the patient to quit smoking, including e-cigarettes, as it can interfere with healing.

During the Procedure

Upon arrival at the hospital, patients will be asked to change into a hospital gown. The doctor will administer either spinal anesthesia to numb the lower body or general anesthesia to put the patient to sleep.
The surgeon may also inject a nerve block or medications around the joint to manage post-surgery pain.

Steps of the surgery typically include:

  • Making an incision above the hip through tissue layers
  • Removing the damaged bone and cartilage, preserving healthy bone
  • Inserting a replacement socket into the pelvic bone
  • Placing a metal stem into the top of the thigh bone, which is topped with a replacement ball

After the Procedure

After surgery, the patient will be moved to a recovery area for several hours until the anesthesia wears off. The medical team will monitor blood pressure, heart rate, alertness, pain level, and medication needs.

Patients may be asked to breathe deeply, cough, or use a breathing device to help clear fluid from the lungs.
The duration of stay depends on individual needs, but most patients can go home the same day.

Hip Replacement Recovery Outcomes

Full recovery varies by patient, but most improve within about three months after surgery. Progress continues throughout the first year.

A new hip joint can relieve pain and improve mobility, but patients should not expect to do everything they could before hip pain began.

High-impact activities such as running or playing basketball may strain the artificial joint. However, most people can return to low-impact activities like swimming, golfing, and cycling.
