Vaksinasi Covid-19 Pasien Komorbid, Apa yang Perlu di Perhatikan ?

Apa kriteria untuk menerima vaksinasi Covid-19 pada pasien dengan komorbid ? Sebelum itu kenali dulu komorbid adalah penyakit penyerta. Dikutip dari Britannica, komorbid merupakan penyakit atau kondisi yang muncul secara bersamaan pada individu/orang. Gangguan komorbiditas biasanya lebih parah, kronis, dan sulit diobati daripada gangguan murni.

Melansir Science Direct, komorbid dapat disebabkan oleh setidaknya tiga kelas. Pertama, ketika satu gangguan secara langsung memengaruhi permulaan gangguan kedua. Misalnya, penyalahgunaan alkohol terus-menerus dapat menyebabkan sirosis hati. Proses kelas kedua melibatkan efek tidak langsung dari satu gangguan pada permulaan gangguan kedua. Contohnya, tekanan terkait ancaman dan perubahan gaya hidup setelah didiagnosis penyakit jantung dapat memengaruhi timbulnya gangguan kecemasan. Pada kelas ketiga, komorbid dapat melibatkan penyebab umum. Peristiwa traumatis, misalnya, dapat memengaruhi berbagai macam kecemasan komorbiditas dan gangguan mood.

Berdasarkan rekomendasi PAPDI (Perhimpunan Dokter Spesialis Penyakit Dalam Indonesia), vaksinasi COVID-19 (CoronaVac) dapat diberikan pada pasien komorbid dengan ketentuan :

  1. Penderita DM Tipe 2, vaksinasi COVID-19 layak diberikan apabila kadar gula darah terkontrol dan nilai HbAlc di bawah 7,5%.
  2. Penderita hipertensi, vaksinasi COVID-19 layak diberikan jika tekanan darah <180/100 mmHg dengan atau tanpa obat.
  3. Penyakit paru, (contoh : PPOK (Penyakit Paru Obstruksi Kronik) dan asma) vaksinasi COVID-19 layak diberikan jika kondisi stabil dan terkontrol.
  4. Penderita TBC, vaksinasi COVID-19 layak diberikan apabila sedang dalam pengobatan OAT (Obat Anti Tuberkulosis) minimal 2 minggu.
  5. Penderita PGK (Penyakit Ginjal Kronik) non dialisis & dialisis, vaksinasi COVID-19 layak diberikan apabila kondisi umum stabil (tidak sedang mengalami komplikasi akut terkait PGK atau mendapat penilaian layak menerima vaksinasi COVID-19 dari dokter yang merawat).
  6. Penyakit jantung (gagal jantung dan penyakit jantung koroner),vaksinasi COVID-19 layak diberikan jika kondisi stabil dan terkontrol.
  7. Penderita penyakit autoimun sistemik, vaksinasi COVID-19 belum layak diberikan pada penyakit SLE (Systemic Lupus Erythematosus). Sjogren. RA (Rheumatoid Arthritis). dan vasculitis. Sedangkan pada penyakit tirold autoimun. penyakit autoimun hematologi. dan IBD (Inflammatory Bowel Disease) yang terkontrol. vaksinasi COVID-19 layak diberikan.
  8. Penderita HIV, vaksinasi COVID-19 layak diberikan apabila nilai CD4 > 200.

Perlu diingat. Rekomendasi PAPDI saat ini masih terbatas pada Vaksin CoronaVac saja.

Mental Health During The Covid-19 Pandemic

Eight months have passed, yet COVID-19 cases in Indonesia are still increasing and causing the public to be restless. kawalcovid19 reported that the total confirmed cases of COVID-19 as of October 7, 2020 totaled up to 315,714, with 240,291 cases declared cured.

The Indonesian Psychiatric Association (PDSKJI) noted that in the 5 months since the COVID-19 pandemic began, 64.8% of psychological disorders had occurred. The most common psychological problems are anxiety, depression and psychological trauma. Of the 4010 people included in the study, 71% were women.

The various psychological problems that have occurred during the COVID-19 pandemic are thought to be associated with lifestyle changes and widespread information about the dangers of COVID-19. The Indonesian government’s policy, Large-Scale Social Restrictions (PSBB, has affected the public’s lifestyle. The policy states that activities outside the home must be limited; in fact, almost all employees are required to work from home. Given these limitations, many people have difficulty adapting and show different responses. According to the viewpoint of a psychiatrist, with regard to facing COVID-19, each person exhibits the following two responses:

  1. Reactive is the attitude of someone who responds to a situation with distress, exhibiting tension, aggression, and panic.
  2. Responsive is the attitude of someone who responds to a situation with eustress, staying calm and wise, and learning about the condition in a proper manner.

Psychological problems such as anxiety, depression, and psychological trauma generally result from reactive responses. The World Health Organization provides several tips for maintaining mental health during the COVID-19 pandemic, namely:

  1. Seeking out trusted friends or family to chat and share your feelings
  2. Maintaining a healthy lifestyle, for example eating nutritious foods, getting enough sleep, exercising, and still socializing with friends or family who are not living in the same house
  3. Not smoking or consuming alcohol and illegal drugs
  4. Seeking information about COVID-19 from reliable sources
  5. Limiting yourself from accessing information about COVID-19 that makes you feel anxious or afraid
  6. Engaging in hobbies to maintain emotional stability

If you feel that your anxiety or fear is interfering with your daily activities, seek immediate help from the nearest psychiatrist or psychologist.

What criteria determines that I’ve been “cured” from COVID-19?

According to the August 2020, 2nd edition of the COVID-19 Management Guidelines, a person who is confirmed to be infected with COVID-19 (with mild-moderate-severe symptoms or without symptoms) is declared “cured” if he or she has met the isolation criteria and is given a statement of completion of monitoring based on an assessment by the doctor in charge.

However, in someone who is confirmed to have been infected by COVID-19, there is a possibility that the results of the follow-up RT-PCR will be positive again. Consequently, the determination of “cured” cases is made based on assessments by the doctor in charge, and can change at any time during the pandemic.

What is Post-COVID-19 Syndrome?

This condition occurs due to organ damage caused by the SARS-CoV-2 virus. Data from several studies show that the greatest impact of COVID-19 infection occurs in the lungs. The same thing has also been reported in previous coronavirus infections, namely SARS and MERS, which have been known to persist for up to 7 years.

What is New-Illness Related Fatigue?

In COVID-19 infections, the SARS-CoV-2 virus that enters the body will cause immunological reactions and interfere with many cell functions in the body, especially the lungs. Changes in the shape of cells in the lungs will cause the oxygen exchange function to be disrupted. Generally speaking, lung changes found post-COVID-19 infection will fall into the category of restrictive lung disorders. These pulmonary changes will affect the respiratory process and cause someone who has been infected with COVID-19 to tire more easily. This is commonly known as new-illness related fatigue. A study in hospitals in Europe found that new-illness related fatigue can occur in patients of a very wide age range, from 20 to 84 years old.

What should I do if I have Post-COVID-19 Syndrome?

Rehabilitation is an aggregation of medical efforts to return the body’s condition back to its pre-disease state, corresponding to the patient’s ability. One of these efforts is physical exercise, which aims to maintain or improve current organ function. Physical exercise is highly recommended as a way to maintain or restore the patient’s quality of life back to the way it was before being infected with COVID-19, while adjusting to changes in the anatomical structure that occurred due to the infection.
